This is wordy. Can you say this more directly and with less jargon? Below, I will strip away much of what you wrote in an attempt to find the "heart" of the sentence. When I do, I find that "theory" might not be the right word.
This "theory" helps patients and their families meet their self-care deficit needs until they are capable of independently meeting those needs.
I hope I have not changed your meaning too much.
