Creative opinions, please

I have a couple question regarding the following sentence:

   Hope would have been her name, had she been given the opportunity to be.

1.  Comma or no comma?

2.  How many forms of 'to be' can one have in a sentence?  I tried 'to live', but it didn't work for me.
