You have two versions of the same sentence. The first is correct.  The second is correct if 'be' is removed.  You can remove 'in order' from either sentence to make it shorter.

 

Healthcare providers, patients, communities, and researchers must be actively working to develop creative, individualized programs in order to improve healthcare for minorities.
