Addressing a formal letter to a physician and his wife

What is the proper way to address a formal letter to a physician and his wife. For example, should it say; Dr. And Mrs. John Doe?

I would use 'Dear', as well.  'And' should not be capitalized.

 

Dear Dr. and Mrs. John Doe,
