The first problem is that you worded it one way and then tried to word it both way and then neglected to finish making the change.  A bug in Grammarly is that when you highlight a text, you cannot type over it.  So if you are not watching, you are adding the new to the old and making a bigger mess.

 

So the first order of business is to remove ", that the therapist will should be aware."

 

Then there is the missing helper verb toward the beginning.  "It is important that Abby [is] aware."  To word that shorter, "It is important that Abby knows..."

 

The ending can  be changed from "...without her conscience and approval" to "without her consent."

 

So far, that leaves this:

 

It is important that Abby is aware that the therapist will ensure confidentiality of information about her treatment and that no information will be released to any third party without her full consent.

 

That still leaves 33 words and without punctuation.  That is still wordy, but more understandable.  Now we should cut out some of the redundancy.  The clauses in the middle can be combined since ensuring confidentiality and not releasing information mean the same thing.

 

It is important that Abby knows that the therapist will not release any information about her treatment to any third party without her full consent.

 

We could cut it even more if we assume that the only place any information could be released would be to  a third party.

 

It is important that Abby knows that the therapist will not release any information about her treatment without her consent.

 

There it is in 20 words.

Abby is comfortable when she knows the therapist will only release confidential treatment information upon Abby's explicit conscent.
