The singular form of "know" is correct:

 

"Only one in five people knows the answer to this question."

 

In the sentence, "in five people" is a prepositional phrase. Pull it out, and it's easy to see that "knows" is correct because it corresponds with the singular "one": Only one knows the answer to this question.
