Continuation of a sentence after a question mark and closing dialogue

I am unsure how to continue after a question mark in dialogue, for example:-

 

"Are you sure?" she asked, surprised at his demeanour.

 

Grammarly wants a capital S for the start of a sentence, but this is not the start of a sentence.

Penny,

 

You always start the beginning of a quote with a capital letter, unless it is a continuation of the quote in the sentence, for e.g.:

 

"Are you sure? I mean, if that's what you really want," she said, surprised, "then I can't stop you."
