A- Only when do you become a parent, you will understand what true responsibility is.
B- Only when you become a parent, will you understand what true responsibility is.

"B" is closer to being a correct sentence, but here are some correct options:

 

Only when you become a parent will you understand what true responsibility is. (no need for a comma in this sentence)

 

You will only understand what true responsibility is when you become a parent.

 

Becoming a parent is the only way to understand what true responsibility is.

 

Becoming a parent is the only way to truly understand what responsibility is.

 

You won't/won't be able to/can't understand what true responsibilty is until you become a parent.
